Supported accommodation is a precious resource and we must ensure that it is allocated fairly, and to those in greatest need. With the exceptions of Direct Access and Sheltered Housing, Supported Accommodation in Fareham & Gosport is accessed via the Supported Housing Panel (SHP).
The process of allocating supported accommodation was amended in October 2010. All cases requiring supported accommodation must be referred to the SHP in the first instance so their case can be assessed.
The information held on this web-page sets out how the Panel operates, and how referrals can be made. If you are a practitioner and wish to refer your service user to the Panel, then please request a referral form by emailing supportedhousingpanel@fareham.gov.uk.
Accommodation Providers should not arrange any placements into their schemes without adhering to the Nomination Procedure. Nominations are presently managed by Gosport Borough Council and related enquiries should be made to supported.housing@gosport.gov.uk
Referrals for floating Support can be made directly to a support provider and do not need to go through the SHP.
The Panel will seek to identify suitable pathways into Support for each case referred. Once this has been completed, referring agencies may need to complete a further referral to the identified support scheme for assessment and approval.Fareham Borough Council, Civic Offices, Civic Way, Hampshire, PO16 7AZ
