How to Apply for a Bus Pass or Travel Tokens

You may apply for a bus pass or travel tokens in person at the Civic Offices on any weekday during normal office hours. .

How to apply for a bus pass

Step 1: Complete an application form for a Fareham Bus Pass.

Application forms are available:

Step 2: Provide evidence of your age.

Evidence of your age include:

  • birth certificate
  • passport

Step 3: Provide evidence of your disability (if applicable).

Evidence of your disability include:

  • blind registration certificate
  • statement of disability living allowance showing the higher rate of the mobility component
  • a medical statement explaining your disability. A standard form is available but your doctor may charge you for this certificate.
  • other evidence supporting your disability e.g. a certificate from your doctor

Step 4: Provide evidence of your address

Evidence of your address include:

  • Fareham Borough Council Tax Bill
  • Utility Bill
  • Driving Licence

Step 5: Return your application and relevant documents in person.

Visit the Council's Customer Service Centre in the Civic Offices, Civic Way, Fareham on any weekday during normal office hours, with all your relevant documents. A digital photograph will be taken of you and your pass will be ordered for you. You can expect to receive your pass in the post within 5 working days.

 

If you cannot attend in person please contact the Customer Service Centre on Tel: 01329 236100 to discuss your circumstances.

 

If you are over the age of 16, you will be asked to provide your National Insurance Number.

 

How to apply for Travel Tokens

Step 4: Return your application with relevant documents in person.

Visit the Council's Reception in the Civic Offices, Civic Way, Fareham during normal office hours, with all your relevant documents. A photograph will be taken of you and an identification card and your tokens will be issued straight away.

 

If you cannot attend in person please contact us to arrange a suitable alternative..

 

You will be asked to provide your National Insurance Number.

Renewal of your bus pass or travel tokens

Renewal of Your Travel Tokens

All token holders with expiry dates between 1 April 2010 and 31 March 2011 will receive a letter detailing the new scheme, around one month prior to the renewal date.

 

Applications that are received more than one month after the expiry date will only be effective from the date of application i.e. they will not be back dated.

 

Your application should be made in person. Please visit the Council's Reception in the Civic Offices, Civic Way, Fareham on any weekday during normal office hours.

 

If you are unable to visit the Civic Offices, please contact us to arrange a suitable alternative.

 

You will be asked to sign a declaration confirming your continued eligibility to the concession.

Renewal of Your Fareham Bus Pass

Please contact the Concessionary Travel Office one month before the expiry date of your current pass to request a renewal application form.  As the renewal process will be undertaken by post, please ensure that you inform the Concessionary Travel Team if you change address.
