The Exercise Referral Scheme has been set up to encourage and support you in carrying out regular exercise as a habitual part of your life, which can benefit your heart, weight, muscles and mind. Regular exercise can help protect you from developing health problems or assist you to manage and control existing problems.
This scheme has been developed in partnership with Fareham Borough Council, Fareham Leisure Centre (Everyone Active) and Hampshire Primary Care Trust. Further information can be found on the Exercise Referral Scheme leaflet (980 kb)

Important note- if you would like to be referred on to the scheme, please discuss it with your GP first, they will then be able to make the necessary referral to the scheme administrator.
